The Maine YAC is a part of a grant from the government called Integrated Services for Children and Youth with Special Health Care Needs in Maine. Here are the Goals & Objectives, you can also find out more about IS on the website - www.servicesforme.org.
- Goal 1:
Youth with special health care needs and their families will collaborate in decision making at all levels and will be satisfied with the services they receive. - Goal 2:
All children and youth with special health care needs will receive coordinated, ongoing, comprehensive care within a medical home. - Goal 3:
All children and youth will be screened early and continuously for special health care needs. - Goal 4:
All youth with special health care needs and their families will have sufficient private and/or public insurance to pay for the services they need. - Goal 5:
Services for children and youth with special health care needs and their families will be organized in ways that families and youth can navigate and use them easily. - Goal 6:
All youth with special health care needs will receive the services necessary to make appropriate transitions to all aspects of adult life, including adult health care, work, and independence.
